• Fixed one holder's dispose() tearing down the shared CardanoLedger.ble() / CardanoLedger.usb() instance while other holders were still using it (LedgerManagerDisposedException). The shared instance is now reference counted and only disposed when the last holder disposes it. Every ble() / usb() call must be balanced by exactly one dispose().
• Extra dispose() calls on an already released instance are now no-ops and never clear a newer shared instance
• Added re-exports of the ledger_flutter_plus exception types (LedgerException and its subtypes, UnexpectedDataPacketReason) plus UniversalBleException and PairingException, so consumers can type-check connection errors without depending on ledger_flutter_plus directly
• Updated ledger_flutter_plus to ^1.6.1 (fixes uncaught async errors in BLE send, scan and scan cancel)
