Turning a selection into a comment range and back: $markSelection, $removeMark, $getMarkedText, $getMarkIdsAtSelection and the two commands behind them. Marking a range that already carries a mark adds the identifier to the mark that is there rather than stacking a second one, so the common case stays flat; a mark left with no identifiers is unwrapped, because otherwise every resolved comment would leave an element in the document forever.
