ModelInfoMakeRequest class

Inputs for the canonical RAModelInfo factory. Replaces Swift's RAModelInfo.make(...) ~370 LOC of field-defaulting and artifact-inference logic with a commons-owned implementation. Commons fills 18 ModelInfo fields (id, name, category/format/framework defaults, context-length defaults, thinking gating + default pattern, artifact inference, source mark, timestamps, and is_downloaded probe).
