A fetch failed for good: retries exhausted, or cancelled with neither
silent nor revert — a reverting cancel restores the earlier state
through QuerySetStateAction instead. Moves status to error, ends the
fetch and flags existing data as invalidated. TanStack Query calls this
action error.
