Instacart-style address form.

NO Scaffold. NO AppBar. Host provides both. NO door/building photo (removed per design spec).

Riverpod state: plugFormProvider — all form fields + PIN auto-fill plugSavedAddressesProvider — save / update / delete plugSelectedAddressProvider — updated after save

After save, host can read:
