MssqlNulls enum

Where SQL Server should put NULLs relative to other values.

SQL Server has no NULLS FIRST / NULLS LAST syntax. The default treats NULL as the lowest value, so ASC is nulls first and DESC is nulls last. An explicit value compiles to a CASE ahead of the real term.
