FolderModelState class final

The model/provider triple saved for one project folder.

FolderModelState({required String providerKind, required String modelId, required String? baseUrl, String? customProvider})

Creates a FolderModelState. customProvider is the saved custom provider ENTRY NAME the triple came from (gh-1000): a restore pins that entry instead of re-matching by endpoint, so two entries sharing one endpoint+modelId restore onto the right account's key.
