ModelChangeRecord class final

Records a change of the active model.

Ported from pi's ModelChangeEntry. baseUrl/customProvider pin the serving endpoint and saved custom-provider entry (gh-1000): a session restore re-resolves onto THAT entry instead of re-matching by model id. Both are optional — records written before gh-1000 restore with today's endpoint-keyed behavior.
