DiagramDocumentChanges class final

Canonically proven changes between immutable document snapshots.

added and replaced hold current instances; removed holds previous instances. A replacement preserves its identity but may change any content. reorderedIds identifies surviving elements affected by explicit reordering; insertion/removal does not report every element whose numeric index shifts. Consumers always obtain current positions from DiagramDocument.indexOfElementId.
