DateTime? get lastRefreshAt

The timestamp of the most recent successful refresh, or null if no refresh has completed in this keeper's lifetime.

Updated every time _executeFlight stores a new token — including refreshes triggered by forceRefresh, getValidToken, and the TokenKeeperInterceptor 401 retry path.
