KeyEventReader class

Synchronous key event reader for raw terminal input.

Wraps TerminalContext.input.readByteSync, interprets multi-byte escape sequences, and emits normalized KeyEvent instances. Works in tandem with TerminalControl.enterRaw / TerminalModeState so prompts can switch in and out of raw mode safely.
