RangeDigestBuilder class

Streams records into a RangeDigest: an engine calls add once per record in the range (any order) and takes build.

A record's identity is its (key, hlc) pair — the HLC changes whenever the value does, so equal pairs mean equal record versions. The fingerprint is the XOR of the first 16 bytes of SHA-256(key ‖ 0x00 ‖ hlc): order-independent, so ranges can be summarised in one streaming pass and split without re-hashing.
