OrSet class final

Observed-remove set: concurrent adds and removes of different elements both survive a merge, and an add wins over a concurrent remove that did not observe it.

The field holds a list. Every add is tagged with an id unique to the device that made it; removing an element tombstones the add ids the remover could see, so an add it never observed survives. The resolved list is de-duplicated and sorted by the elements' JSON encoding, so converged replicas read identical lists.
