ToolFailure class

Thrown by AiChatTool.run to hand the model a specific reason for failing.

Any other exception is reported generically instead: it may carry internals -- urls with tokens, schema, file paths -- that shouldn't reach the model, let alone the conversation history summarised from its answer.
