SilentStreamPolicyHooks class final

The escalation hooks the host/resolver wires (the roles chain owns the key rings and the smol role; this file stays below model_roles/).

Creates the hooks. All-const default = no escalation available (the ladder then only backs off and, past its budget, stands the error).
