misuseBreaker property
The tool-misuse circuit breaker switch (agent.misuseBreaker,
issue #862, default true): 3 consecutive identical tool-call
rejections arm a corrective note in the next request; 6 stop executing
that identical call for the run. false preserves the pre-breaker
behavior byte-identically (E5).
