reportedOrphanKeys property

Set<String> get reportedOrphanKeys

gh-1449: the session's one-shot latch of already-reported orphan results (orphanReportKeys, mutable — the agent loop adds every key it reports back into this same set). Hosts seed it from the persisted orphan_report custom records when a session resumes (orphanReportKeysFromRecords), so a restored session never re-reports an orphan it already reported (AC6).
