ProviderTuningRegistry class final

The process-wide per-provider tuning table: registry entries that declare connectTimeoutMs/streamIdleTimeoutMs are seeded here at boot. Boot-scoped (review r2): a mid-session config edit does not re-seed by itself — a host applying a fresh config re-runs the seeder with reset: true (E4: the NEXT request recomputes; an in-flight one already resolved its values).
