Duration const providerFetchConnectTimeout

Connect/first-headers watchdog for NON-streaming provider fetches (model lists, OAuth/token endpoints, quota probes): an endpoint that never even answers headers fails fast instead of hanging the session (issue #1036). Tighter than providerConnectTimeout on purpose — a fetch is small, unlike a streamed generation.
