ChunkUploadException class

Exception for chunk upload failures (carries resume state).

With concurrent uploads, chunks complete out of order, so resume can no longer assume "all chunks < N are done". completedChunks carries the exact set of indices that succeeded; lastSuccessfulChunk is kept as the contiguous-prefix high-water mark for backward-compatible callers. fileKey must be carried so a resumed upload reuses it — chunks already on the server were encrypted with it.
