DVUsageMeter class

Something an application counts and a provider bills for.

The meter is the application's name for the thing -- api_calls, seats, minutes_transcribed. What the provider calls it is provider configuration, the same way a plan's price identifier is, so a meter can be referred to from application code without the code knowing which billing account it ends up in.
