InstanceProvider class abstract interface

Supplies secondary instances without reading a file, e.g. from a database (ODK Collect uses this for entity lists).

Port of ExternalInstanceParser.InstanceProvider. Providers are synchronous because a partial instance is completed on demand during XPath evaluation (see ExternalDataInstance).
