McpUrlElicitationRequired class

Thrown by a tool handler when the call cannot proceed until the user completes URL-mode elicitations (URLElicitationRequiredError, -32042, 2025-11-25). Each entry is URL-mode elicitation params with an elicitationId. On other revisions it is reported like any tool failure.
